Viagra is a drug used to treat sexual problems. The drug works to improve the natural mechanisms underlying an erection. Viagra can be used by men suffering from erectile dysfunction of various origins (e.g., vascular, neural disorders, etc.).

The main active ingredient in Viagra is sildenafil, which is a potent and selective inhibitor of cGMP-specific phosphodiesterase type 5 (PDE5).
The physiological mechanism of penile erection involves the release of nitric oxide within blood vessels during sexual stimulation. Nitric oxide activates the enzyme guanylate cyclase, which leads to increased levels of cyclic guanosine monophosphate, relaxation of smooth muscles, and increased blood flow to the penis.

A mandatory condition for the effectiveness of sildenafil is sexual stimulation.
Viagra should be taken orally. For most patients, the recommended dose is 50 mg. Viagra should be taken about 1 hour prior to sexual activity. When taking into account efficacy and tolerability, Viagra dosages can be increased to 100 mg or reduced to 25 mg. The maximum recommended frequency of Viagra consumption is once per day.

Contraindications

  • Hypersensitivity to any component of the drug.
  • Use in patients who receive permanently or intermittently donators of nitric oxide, organic nitrates, or nitrites in any form.
  • Lesser than 18 years of age.
  • Not suitable for use in women.

Viagra should be taken with caution in the following cases:

  • anatomical deformation of the penis (including angulation, cavernous fibrosis, or Peyronie’s disease)
  • diseases increasing the risk for the development of priapism (such as sickle cell anemia, multiple myeloma, leukemia, and/or thrombocythemia),
  • diseases accompanied by bleeding, exacerbation of peptic ulcer, hereditary retinitis pigmentosa, heart failure, unstable angina, suffered the myocardial infarction, stroke or life-threatening arrhythmias, arterial hypertension (BP > 170/100 mm Hg), or hypotension (< 90/50 mm Hg) within the last six months.

To diagnose erectile dysfunction, it is important to determine the possible underlying causes and select an adequate treatment. Therefore, it important to complete a medical history document coupled with a thorough physical examination. Drugs intended for the treatment of erectile dysfunction should not be prescribed to men for whom sexual activity is undesirable.
